The Hammers produced an awful display at the City Ground and deservedly lost by a big margin, with the defeat leaving them just six points above the drop zone.
Chris Wood scored the opener before Edson Alvarez received his marching orders for his second yellow card at the stroke of half-time.
Callum Hudson-Odoi and Ola Aina were also on target as West Ham slumped to a disappointing away loss.
Loeptegui must now guide his side to victory this Saturday when they take on Everton at the London Stadium.
The Spaniard may be out of a job during the international break if West Ham fail to win.

Lukasz Fabianski is set to make his third Premier League start after positive performances in the previous two games.
With Lopetegui reverting to a 4-2-3-1 formation, Konstantinos Mavrapanos could drop out of the starting XI.
The 26-year-old, who joined the club for £19 million in 2023, produced a disappointing display against Forest, with Alex Moreno beating him before setting up their opening goal.
Max Kilman and Jean Clair-Todibo will likely start in central defence, with Aaron Wan-Bissaka and Emerson playing on the flanks.
Edson Alvarez will sit out the game due to suspension, meaning Carlos Soler could partner Guido Rodriguez in midfield.
Lucas Paqueta will operate from the number 10 role, with Crysencio Summerville and Jarrod Bowen playing as the wide attackers.
Michail Antonio featured off the bench last weekend, but Danny Ings will likely spearhead the attack against Everton.
